Setting up an account

If this is the first time you are placing an order with us, you need to set up an account with us. To do so, first read the terms page.  Then send the following infomation by fax or email:

• A purchase order form.
• Your shipping and billing address.
• Contact person's name, phone and fax numbers, and email address.  If there is an alternate contact person, please include that information as well.

Ordering methods:

    Payment terms are net 30 days of invoice date

• Fax.  Send your purchase order to 1-608-567-4031. 
• Email.  Please send your email orders to

                sales@lacrosselabs.com

• Telephone.  You may call us at 1-608-567-4231 to place your order.  Note that we are a small company and there is not always someone available to answer the phone, but you can always place an order by FAX or email.

Payment methods:

We accept check, money order, PayPal and wire transfer.  If you need to use a credit card, please contact us first.


• Check or money order. Please make it payable to

• Wire transfer. Please contact us for our bank information. We add a $25 charge for wire transfer of payment.
